Financial Information

Dr. Frank Palmaccio and the staff at Woodbury Periodontics, PC are proud to be a team whose primary mission is to deliver the finest and most comprehensive periodontal and dental implant services available today. We are concerned about your dental care and want to ensure you that it is performed in the most responsible manner. In order to assist you with the investment in your dental health, we have outlined our payment policy.

Dental Insurance

We will gladly process your insurance claim to help you receive the full benefits of your coverage as a courtesy to you. However, you must complete the insurance section of our New Patient Form in its entirety. We will send a request for pre-authorization/pre-estimate for procedures other than maintenance cleanings but trust that you will check with your insurer to find out the details and/or limits of your coverage. It can take from 1-4 weeks to hear back from your insurance company so we will try to schedule your appointments accordingly. We cannot make any guarantee of estimated coverage or payment but please know that we will do everything possible to see that you receive the full benefits of your policy. You will be asked to pay your deductible and your co-payment for charges on the day the service is rendered. The estimated amount not covered by your insurance may be paid by any one of the other payment options we have available and we will be happy to work with you to plan the most appropriate arrangements for your budget.

Dental insurance is an agreement between you and your insurance company. The ultimate responsibility for all charges lies with you. If after 60 days the insurance company has not paid on the claim, you are responsible for the total charges and will be billed accordingly. We will continue to assist you with these claims to ensure you receive your full benefit. If and when payment from your insurance company is received, we will adjust your account accordingly and you may be due a refund.

Please be aware that we ARE NOT a Medicaid/Medicare provider. As part of our Opt-Out agreement, we must inform you in advance that we will not bill Medicare nor will you be able to submit a claim for reimbursement from Medicare. Should you choose to have treatment in our office, you may take advantage of our other payment options.

Emergency/Single-Visit Payments

For single-visit procedures, our policy is that payment is due in full at the time of service. For your convenience we accept cash, checks, money orders and credit card payment (Visa, Master Card, Discover and American Express), You may also consider payment options through CareCredit.

Multiple Visit Payment Options

Option 1: Payment in Full by Cash or Check at time of service. Please speak with our financial coordinator regarding courtesy discounts that may be available with this option.

Option 2: Payment in Full by Credit Card (Visa, Master Card, Discover and American Express) at time of service. Please speak with our financial coordinator regarding courtesy discounts that may be available with this option.

Option 3: Outside Financing – CareCredit™. CareCredit® is a health, wellness, and personal care credit card that helps patients and clients get the financial help and care they want and need, without delaying appointments or treatment. Speak to our financial coordinator to apply today. We would be happy to process the application for you or you may choose to apply online. If approved, you will receive a credit card via US mail from CareCredit within 7-10 days. Upon receipt, you will need to contact our office to authorize the transfer of your account balance to your CareCredit account. Types of Payment Accepted

For your convenience, payments may be made with cash, personal check, bank drafts, and money orders, Visa, Master Card, Discover and American Express. If you have any problems or questions, please contact our staff. They are well informed and up to date. We can be reached at 631-629-4760.

Appointment Cancellation Policy

·       Please remember that it is your responsibility to keep your appointment time.

·       As a courtesy, our office may call, text or email you to confirm your appointment time and date. However, if you do not show for your scheduled appointment because you did not receive the courtesy confirmation, it will be treated as a No Show/Broken Appointment and a charge may be applied to your account.

·       Cancellations are not accepted by leaving a message or voicemail. Please call during business hours and we will be happy to assist you.

·       If you are unable to keep your appointment with us, please call us at least 2 business days prior to exam and non surgical appointments. This courtesy allows us to offer your original time to another patient that needs to be seen.

·       If you are unable to keep your procedure/surgical appointment with us, please call us at least 5 business days prior to your appointment date.

·        After 3 missed/broken appointments, Woodbury Periodontics, PC reserves the right to dismiss you from our practice.
